Includes all types of curtains, draperies, venetian blinds, window shades, roman shades, and rollup shades; ready made or custom made, installed or not installed. Excludes curtain and drapery hardware such as brackets and rods. Living Room, Dining Room or Kitchen FurnitureIncludes:
Includes couches, futons designed as both sofa/couch and sleeping surface, convertible and non-convertible sofas and love seats, both singly and in sectional/modular groupings. All types and styles of living room chairs: such as recliners, upholstered lounge chairs, tub chairs, club or state chairs, wingback chairs, bentwood chairs, contoured chairs, desk chairs, directors chairs, bean bag chairs, rockers or rocking chairs, folding chairs, swivel chairs, metal base chairs, wicker chairs, inflatable chairs and armless chairs. Living room tables: such as coffee tables, cocktail tables, end tables, and all sizes, materials, and shapes of lamp tables. The specialty of the physician could be any of the following: allergy, cardiology, child psychiatry, endocrinology, family practice, general practice, general preventative medicine, general surgery, gynecology, hematology, internal medicine, laryngology (throat, pharynx, and larynx), nuclear medicine, obstetrics, oncology, orthopedics, otology (ear), otorhinolaryngology (ear, nose, pharynx, and larynx), pathology, pediatrics, psychiatry, psychoanalysis, radiology, rhinology (nose), abdominal surgery, cardiovascular surgery, neurological surgery, orthopedic surgery, pediatrics surgery, plastic surgery, thoracic surgery, and urological surgery. Excludes:
Dental ServicesIncludes:
Includes dental services such as: exams, cleanings, x-rays, orthodontic treatment (braces), dental sealants, consultations, bridges, dentures, impacts, extractions, oral surgery, restorations (fillings and crowns), root canals, treatment of periodontal disease, bonding, and TMJ treatment, as performed by dentists, oral or maxillofacial surgeons, orthodontists, periodontists, endodontists, or other dental specialists in group or individual practice. Also includes all types of hearing aids and the cost of testing and fitting of the hearing aid. Excludes:
